Purpose and Consent

For attention of Parent/Carer if under 16 years or the young person if age 16 years and over

Purpose:

  • The seeking of information between agencies is an important part of the assessment, as it provides a fuller picture of the young person's strengths and needs.  Sharing information allows for a range of specialised assessments to be undertaken to help determine the needs of the young person. In order for a full assessment to be undertaken, several agencies may need to become involved.​​​​​​​​​​​​​

Consent:

  • I understand that the information provided on this form will be processed in accordance with the requirements of the Data Protection Act 2015.  It will be treated as confidential and will only be used for purpose of the provision of education and health services. In connection with this purpose, the information may also be processed for the purpose of preventing any fraud or criminal offence to ensure the health, safety and welfare of any young person. In pursuit of these legitimate purposes, the information may be shared with other authorities, and with any organisation legitimately investigating allegations of fraud, criminal offences or child protection. This information may be stored on a secure database, and anonymised information may be used for audit purposes.
  • I consent for information sharing between the pathway and the agencies named, and for the young person to be referred to services that are deemed appropriate by the pathway, based on my child’s needs.
  • I understand that the pathway will refer the young person to services that will be of benefit to them and these assessments are essential to providing a full and holistic picture of the presentation of the young person. By giving this consent, I am happy for the young person to be considered for assessment and understand this referral will be discussed at a meeting of professionals in order to provide the young person with the support best suited to the young person's needs.  I agree, wherever possible, to arrange for the young person to attend all appointments sent out and understand that non-attendance can lead to the young person being discharged from that service, this will result in an extended waiting time for assessments, and may result in the young person being closed to the pathway.
  • It is important that you are aware that at the age of 16 your child has a legal right to access our services independently and confidentially. For under 16's there are also some rare situations where your child may access our services without your knowledge. The CYP Mental Health Hub are required to speak directly with young people who are aged 16-18 years old to process their referral further. Once initially contacted, they can give specific permission to speak with a parent/carer on their behalf.
